Once you subscribe to Kindle Unlimited you will often find the promised "free audiobook companion" cost extra. This issue has been reported numerous times. Since you can't send customer service a picture like below, it takes a long time, but eventually they will reproduce the issue and agree it's an Amazon problem. They will apologize, maybe give you a credit to purchase a book marked included, promise to report the issue and followup... never to be heard from again. (example email below).

Do we have any way to get an Amazon Executive involved in addressing this issue so those who are visually impaired or just like to enjoy an audiobook can get what they paid for?

You can also cancel Kindle Unlimited if it does not meet your needs.

Thanks for your kind reply. Unfortunately, many may have a prepaid membership. Mine is for 2 years. There was/is no way to know before subscribing that "included" audio would be inaccessible. There was no reason to suspect this issue wouldn't be resolved before the credit card dispute period expired.

Amazon Policy: "If you cancel a Kindle Unlimited membership... you will not receive a partial refund for unused months and you cannot restart your membership later.
