Have you filed a complaint with FINRA (http://www.finra.org/investors/file-complaint)? That's the entity that regulates brokers like Fidelity. Given how old this issue is (roughly nine years), it's going to be a tough slog.

One question: you seem to know exactly when you requested these various transactions. How do you know the dates you requested these trades with such specificity? For some honest feedback, it seems odd that you would have records of the exact date you requested each trade, even nine years later, but wouldn't have looked at any of your trade confirmations at the time, or any of your brokerage statements in the years since.
